We are committed to understanding who our service users are, their unique set of care and support needs, personal preferences, and goals.
With this in mind we look to create and deliver effective packages of care, tailored to the individual, ranging from four hours a day up to 24 hours a day, seven days a week.
We work in collaboration with the individual, family members, and professionals from Cornwall Council and the NHS, to ensure a fully holistic approach to support provision for various health and social care needs.
With an emphasis on enablement, and rehabilitation we can support an individual to be as independent as possible in everyday activities and enable participation in education, work, recreation and meaningful life roles. Rightnurse can work alongside physiotherapy and occupational therapists to provide a package of care tailored to meet an individual’s needs and goals.
Working in collaboration with the NHS learning disability teams, consultants in positive behaviour support, and all-important family members, at Rightnurse we provide person centred support to empower individuals to live a fulfilled life by understanding learning disabilities as well promoting health and wellbeing including nutrition and physical health and lifestyle choices.
Whether someone is in the early stages of a dementia or Alzheimer’s diagnosis or has experienced these conditions for some time, we appreciate the need for individuals to maintain their sense of self and independence. Rightnurse can provide the support needed to continue to live at home and within there community.
As a nurse led organisation, Rightnurse are able to supply the clinical care required to support individual’s as they reach the end of their life, assisting them to continue to reside at home and within their community , and often providing much needed respite for family members.
At Rightnurse we can supply support to individuals with mild, moderate or severe mental health conditions. We work with Community mental health teams CMHTs to support people with mental health problems living in the community, with their families and carers providing support with all areas of life, from completing daily living tasks to achieving longer term aspirations.
Whatever someone’s health or social care needs Rightnurse can support individuals to continue to be active within there local community. We can provide support to attend events social activities hobbies education employment and engagement within their neighbourhoods and beyond.
